As a House of Faith volunteer, I agree to promptly report any reasonable suspicion of abuse or neglect to the Department of Human Services (DHS) by calling the Oklahoma.
Abuse & Neglect Hotline at 1-800-522-3511, as required by state law. I understand
that failure to report suspected child abuse or neglect or interfering with such
reporting may result in a misdemeanor charge, and knowingly failing to report
ongoing abuse or neglect for six months or more may lead to a felony charge.
By adhering to these guidelines, I commit to protecting children and vulnerable
adults and fostering a safe environment at House of Faith.
